Family Foundations/Early Head Start
POSITION: Nurse SALARY: $53K annually + benefits
REPORTS TO: Program Coordinator and Health Coordinator
FUNCTIONAL DEFINITION: This team member is responsible for overseeing
maternal and child health, and general family health by attending home visits, providing
health education to parents and staff, and by acting as a community health liaison.
DUTIES INCLUDE:
1. Prioritize and provide selected nurse visits to families in East End and Hill
district.
2. Make home visits which will include:
• Supportive counseling and teaching regarding pregnancy, labor, delivery,
postpartum, family planning, infant care, nutrition, immunizations.
• Assessment of infant’s health, mother mental health (i.e., post-partum
depression) and physical health.
• Teaching about infant feeding, oral health, child development, etc.
• Addressing medical concerns and related teaching for children and family
members and helping families to understand their health needs.
3. Collaborate with Home Visitor, Child Development Specialist, Disability
Coordinator, and Health Coordinator regarding follow up care for any medical
conditions, developmental concerns or Early Intervention as needed.
4. Refer families to appropriate health service providers in the community and/or act
as liaison to advocate or explain mental or physical health needs.
5. Promote health education by:
• Providing in-service and consultation to EHS team members on individual
and family health topics
• Assisting or leading parent groups and meetings by presenting health
education topics.
6. Complete or assist a team member in completion of the health section of new
enrollments.
7. Attend at least two team meetings per site each month. Interpret and clarify
identified health problems and provide consultation to staff on families’ medical
needs/concerns.
8. Participate in infant mental health discussions and IMH meetings monthly.
9. Formally communicate pertinent health concerns with Program Coordinator on a
bi-weekly basis.
10. Collaborates with the Health Coordinator regarding all EHS health data
collections, release of information and data management systems.
QUALIFICATIONS:
• Must be licensed Public Health Nurse, Registered Nurse, or Licensed
Practical Nurse
• Must have experience in working with families, pregnant women, and
young children in low-income neighborhoods.
• Must have experience with working with immigrant and refugee families.
• Must have ability to work collaboratively with others.
• Must have Act 33/34, FBI and Megan’s Law clearances.
• Must provide proof of the COVID 19 Vaccination or medical exemption.
